When you attend your first Prenatal Fitness/Aqua/Yoga class with Fit 4 Two®, you will be asked to complete our prenatal intake form. You can either:
Arrive 10 minutes early to complete the prenatal intake form
or
Download the prenatal intake form, complete it and bring it with you to your first class
